Thank you for your interest in our Instagram Memorial where our SUDC family community can share their child’s story and raise awareness of SUDC.

What Is It?

Our Instagram Memorial page is a loving tribute in memory of children affected by sudden unexpected or unexplained death in childhood.

How does it work?

We collect the following items from you and create an instagram post.   

  • Your child’s story, which is best told by recalling early moments and milestones, personality traits, hobbies, favorite things, and other cherished memories that you’d like to share with others.
  • Photographs of your child

How do I provide my child’s story?

If I have an SUDC Memorial Page, do I need to submit my child’s story here also?

If you would like their story and photos to be posted on our instagram memorial – yes. But rather than starting from scratch, you can take text and photos from your memorial website and send it to us.

If I don’t have an Instagram account can I still participate?

You can still participate if you don’t have an Instagram account. If you would like to create an instagram account, click here and follow the steps.

Photo & Story Consent Form

Please click below to provide your consent to allow the Foundation to use your story. You must be registered with the SUDC Foundation and have completed the intake process. Note, that if you are not the child’s parent or legal guardian, you will need their approval to submit their child’s story for publication. Whether you have a photo consent form on file with the SUDC Foundation or not, we ask you to please complete this consent form to participate in the Instagram memorial and other awareness uses as above. This consent form is more comprehensive as it is inclusive of your story and any specific picture(s) you may want to include.

Write your child's story (400 characters maximum). It will be public, so please do not include addresses, phone numbers and other sensitive personal information. Upon receipt, your story will be reviewed by SUDC Foundation staff and you will be contacted if we have any questions. Your story won't be posted the same day, please allow up to 4 - 8 weeks for our review and note that new stories will be posted weekly on Tuesdays. You will be notified when your child's story is posted.

I hereby authorize the SUDC Foundation, its agents, successors and/or assigns as its interests may appear to use photographs of my minor child and/or their name and/ or their story as below, supplied by me to said organization(s) in any or all of its publicity, including but not limited to, the SUDC Foundation's Instagram Memorial, monthly enews, biannual newsletter, FACES video/slideshow, and/or events and/or literature benefiting or raising awareness of the SUDC Foundation (including those hosted by other foundations or event planners outside of the SUDC Foundation). Said picture(s) may be used with or without using my minor child's given/first name and/or surname. My child's given/first name and/or surname may be used with or without photograph(s). Additionally, I agree that there is no cost to me for submitting my child's photos, name and story for possible publishing and use. I hereby waive any right to royalties or other compensation arising or related to the use of the aforementioned that I provide. I hereby represent and warrant that I am 18 years of age or over. I understand that the SUDC Foundation will make every effort to notify me of my child’s photo or name being used. If the SUDC Foundation wants to edit my story in anyway, I understand that I will be contacted and asked to provide my approval of any edits in writing via email. I agree to indemnify and hold harmless the SUDC Foundation, its directors, employees, volunteers, agents, successors and/or assigns for any and all claims, liability, damages and expenses that may arise from said use of my child's photo, including but not limited to processing, printing, copying or reproduction, in whole or in part, including any claims brought by any other person or entity claiming an interest in the photos or in their subject matter.
