Photo & Name Use Consent Form

SUDC Foundation fundraising events and activities are designed to improve SUDC awareness, engage community advocacy, and raise funds to support the mission of the SUDC Foundation.

These events and activities often highlight the beautiful children lost to SUDC by displaying their photos and/or first name. These precious photographs and first names are entrusted to the Foundation by their parents/legal guardians through written consent. We are grateful to the many families who are helping us raise awareness of sudden unexplained death in childhood in this special way while remembering their beloved child. Click Here to Learn More.
